Washington News: New Credentials Issued With Conditions

Effective October 30, 2008, credentials issued by the Board of Pharmacy include information regarding probation or practice restrictions. A credential with probation or practice restrictions is given at the time of the action and upon renewal. For example, if a pharmacist were on probation, the status block on his or her license would state “Active on Probation.” If a pharmacist’s license were restricted, the status block would state “Active with Restrictions.”
