NEHI Report Highlights Role of Pharmacist in Increasing Medication Adherence and Reducing Hospital Readmission Rates

The importance of medication adherence in reducing hospital readmissions and the role of the pharmacist are highlighted in a new report from NEHI. The report, “Thinking Outside the Pillbox: Improving Medication Adherence and Reducing Readmissions,” indicates that post-discharge drug related adverse events and lack of medication adherence are both factors that drive high readmission rates. The report describes models of discharge planning that include collaboration with hospital and community pharmacists, and that may help hospitals reduce readmission rates.
