Emergency Room Pharmacists Help Reduce Medication Errors
A recent study conducted by Winchester Medical Center in Winchester, VA, shows that involving pharmacists and pharmacy technicians in the hospital admissions process reduced medication errors and improved the accuracy of electronic health records. The results of the study showed that when a pharmacy team interviewed patients in the emergency department about their routine medications, only 48 discrepancies were missed. In contrast, 1,547 discrepancies were missed by the admission nursing staff. This totaled up to 0.3 discrepancies per patient with pharmacist involvement, compared with 8.7 per patient without. The pharmacists contributed to improvements through counseling of patients on such topics as potential drug interactions and long-term use of nonsteroidal anti-inflammatory drugs.
